About the Role
As a Registered Nurse Homecare with VNA Health Group, you will play a vital role in delivering skilled nursing care to patients in the comfort of their own homes across sections of Essex County, NJ, such as Livingston and Roseland. Key Responsibilities:
- Patient Assessment and Care Planning: Assess and evaluate the needs of patients with multiple disease processes and co-morbidities, developing and implementing appropriate plans of care.
- Clinical Documentation: Document all changes in patient condition and update care plans and outcomes accurately and efficiently.
- Patient and Family Education: Identify teaching needs and plan strategies for individuals and families to promote health, prevent illness, and manage disease.
- Quality of Care: Monitor the quality and appropriateness of services provided, ensuring that care meets the highest standards.
- Interdisciplinary Collaboration: Establish and maintain professional working relationships with healthcare providers and other members of the disciplinary teams to ensure comprehensive patient care.
- Infection Control and Safety: Adhere to agency infection control and safety policies, promoting a safe environment for patients and staff.

Requirements and Qualifications
Essential Qualifications:
- Registered Nurse License: Current RN license in the state of New Jersey.
- Clinical Experience: At least 1 year of supervised clinical experience as a registered nurse.
- Communication Skills: Excellent verbal and written communication skills.
- Computer Skills: Strong computer skills, with the ability to document and manage care plans electronically.
- Organizational and Interpersonal Skills: Excellent organizational and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Transportation: Car and valid driver's license, as the role may require travel between patient locations.
Preferred Qualifications:
