Job Summary
As a hybrid remote RN UAS Assessor, you will be responsible for conducting comprehensive assessments of consumers to determine their eligibility for community-based long-term services. You will work independently, utilizing your expertise and knowledge of nursing theory, state, and agency regulations to render medical necessity determinations and resolve technical issues. Key Responsibilities
- Conduct initial assessments of consumers, including evaluation of health status, functional status, strengths, care needs, and preferences.
- Review consumer medical documentation and health referral forms, as relevant to the case.
- Enter assessment data into the UAS-NY online application and transmit, as required.
- Code the UAS assessment correctly based on training and guidelines in the UAS-NY Community Health Assessment Reference Manual.
- Document outcomes of calls to consumers and scheduled appointments.
- Maintain current knowledge of regulatory and project changes pertaining to quality and validity of assessments.
- Enhance communication and collaborative relationships with leadership and interdisciplinary care team members to facilitate receipt of services.
- Review non-qualified assessments with the Clinical Quality Assurance Department.
- Maintain a comprehensive working knowledge of community resources and network services for the target population.
- Perform other duties as may be assigned by the Regional Nurse Manager or other project management.
Requirements and Qualifications
Essential Qualifications:
- Active RN license in the state of New York.
- Minimum 2 years of clinical experience with RN license.
- Minimum of 2 years of experience in a home setting, including work with CHHAs, LHCAs, MLTC, or as a paraprofessional (e.g., HHA, LPN, and/or Private Duty Nursing).
- Active and valid New York State Driver's License or ID.
- Ability to conduct in-home health assessments throughout assigned region.
- Proficiency in navigating web-based applications, Microsoft Outlook & Word, and conducting assessments via Zoom.
- Knowledge of nursing theory, state, and agency regulations.
- Strong organizational, interpersonal, and communication skills.
- Ability to work with multi-generational consumers, demonstrate sensitivity, and perform in a fast-paced environment.
- Capability to manage multiple complex tasks and work independently.
Preferred Qualifications:
- Experience working with telehealth technology.
- Familiarity with community-based long-term services.
- Certification in a related field (e.g., CCM, CRRN).
