Registered Nurse Care Coordination Lead
A company is looking for a Registered Nurse Care Coordination Team Lead (RN) - Population Health.
Key Responsibilities
• Provide clinical oversight and management of the Population Health team
• Lead training and orientation of new staff while monitoring case management productivity and quality
• Collaborate with interdisciplinary teams and lead patient outreach and health promotion services
Required Qualifications, Training, and Education
• Registered Nurse license (required)
• Associate's Degree in Nursing (required); Bachelor's Degree in Nursing (preferred)
• 2 years of Care Management experience
• Certified Care Management Certification (preferred)
• Experience with Epic Healthy Planet and knowledge of population health strategies
