As a key member of our team, you will partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You will work with a diverse population spanning the entire lifecycle, utilizing telephonic and electronic communication methods to empower members to make positive lifestyle and medical choices. **Responsibilities** * Collaborate with a registered nurse case manager to assess, plan, and advocate for members' benefit plan and/or health needs * Research available options and resources for members with financial, transportation, safety, and housing needs * Empower members to make positive lifestyle and medical choices through education and support * Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health * Utilize telephonic and electronic communication methods to communicate with members * Maintain accurate and up-to-date records of member interactions and case management activities * Participate in ongoing education and training to stay current with best practices in medical case management and advocacy **Requirements** * Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence * 2+ years of experience in medical social work or case management * Master's degree in Social Work or related field * Strong communication and interpersonal skills * Ability to work independently and as part of a team * Strong problem-solving and analytical skills * Ability to maintain confidentiality and handle sensitive information * Proficiency in electronic communication methods, including email and phone **Preferred Qualifications** * Experience working with diverse populations, including children, adults, and older adults * Knowledge of community resources and services for medical, financial, and behavioral health * Experience with case management software and electronic health records * Certification in medical case management or a related field **Skills and Competencies** * Strong communication and interpersonal skills * Ability to work independently and as part of a team * Strong problem-solving and analytical skills * Ability to maintain confidentiality and handle sensitive information * Proficiency in electronic communication methods, including email and phone * Strong organizational and time management skills * Ability to prioritize tasks and manage multiple cases simultaneously * Strong attention to detail and accuracy **Career Growth Opportunities and Learning Benefits** At blithequark, we believe in investing in our employees' growth and development.
