[Remote] Home Health Scheduler - Walnut Creek

Health Link Home Health and Hospice is a leading provider of comprehensive home health services throughout the San Francisco Bay Area and East Bay for over two decades. They are currently seeking a self-driven, detail-oriented Scheduling Intake Coordinator to join their Walnut Creek team, responsible for maintaining and coordinating schedules for home health services and assisting with intake processes.
Responsibilities
- Maintain and coordinate schedules for home health services
- Assist with intake and process incoming referral information
- Ensure cases are assigned and staffed within established timeframes
- Communicate schedule updates to clinicians, patients, supervisors, and referral sources
- Manage high call volumes with professionalism and a pleasant demeanor
- Respond to urgent or emergency scheduling needs
- Provide direction to field staff as needed to ensure appropriate patient coverage
- Report scheduling changes and client concerns to supervisors
- Verify time records against schedules
- Follow up on scheduling issues reported by on-call staff
- Attend required meetings and training
- Maintain compliance with agency policies and procedures
- Perform other duties as assigned

- Previous scheduling or intake experience
- Detail oriented with excellent verbal, written, and interpersonal communication skills
- Friendly, professional phone presence and comfortable handling high call volume
- Strong organizational and time-management skills
- Ability to prioritize and multitask in a fast-paced environment
- Strong computer proficiency
- Ability to work independently with minimal supervision
- Willingness to work occasional weekends and holidays as required to support patient care and operational coverage
- 1 year Home Health experience preferred
- Knowledge of medical terminology preferred
