This role will partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. This position will work to empower members to make positive lifestyle and medical choices and will work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health. **Responsibilities** * Collaborate with registered nurse case managers to assess, plan, and advocate for members' benefit plan and/or health needs * Research available options and resources for members with financial, transportation, safety, and housing needs * Empower members to make positive lifestyle and medical choices * Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health * Utilize a collaborative process of assessment, planning, and advocacy to meet members' needs * Develop and implement individualized plans to address members' needs and goals * Provide education and support to members to promote healthy behaviors and self-management * Collaborate with other healthcare professionals to ensure comprehensive care and coordination * Maintain accurate and timely documentation of member interactions and progress **Requirements** * Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in state of residence * 2+ years experience in medical social work or case management * Masters Degree in Social Work (MSW) * Strong communication and interpersonal skills * Ability to work independently and as part of a team * Strong problem-solving and critical thinking skills * Ability to maintain confidentiality and handle sensitive information * Strong computer skills and ability to learn new software and systems **Preferred Qualifications** * Experience working with diverse populations and in a healthcare setting * Knowledge of community resources and services * Experience with electronic health records and other healthcare software * Strong analytical and problem-solving skills * Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities **Work Environment and Culture** * This is a remote work-from-home role, and you will be required to work from a dedicated home office space * You will be working with a team of healthcare professionals who are passionate about delivering high-quality care and making a meaningful impact in the lives of our members * We offer a collaborative and supportive work environment that encourages open communication, creativity, and innovation * We prioritize work-life balance and offer flexible scheduling and remote work options to support your needs **Compensation and Benefits** * The typical pay range for this role is $46,597.20 - $101,228.40 * This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls * The actual base salary offer will depend on a variety of factors, including experience, education, geography, and other relevant factors * This position is eligible for a CVS Health bonus, commission, or short-term incentive program in addition to the base pay range listed above * We offer a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) retirement savings plan, and employee stock purchase plan * We also offer paid time off (PTO) or vacation pay, paid holidays, and sick time, consistent with relevant state law and company policies **How to Apply** If you're a motivated and compassionate social worker who is passionate about delivering high-quality care and making a meaningful impact in the lives of our members, we encourage you to apply for this exciting opportunity.
