Remote CNA / MA Happy Vitals PC Full-Time | Fully Remote Compensation: Competitive, based on experience About the Role Happy Vitals is seeking a Remote CNA/MA to support our Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs. This is a structured, backend clinical support position designed for someone who thrives on accuracy, accountability, and operational excellence in a remote environment. You will play a critical role in reviewing patient health data, ensuring documentation compliance, and supporting our clinical team in delivering high-quality, technology-enabled care. This position works under the supervision of a Registered Nurse and within clearly defined clinical protocols. What You’ll Be Responsible For Patient Monitoring & Data Review • Review daily remote patient monitoring data through designated platforms • Identify abnormal readings based on established protocols • Escalate findings to the supervising RN according to workflow guidelines Documentation & Compliance • Accurately document patient interactions, outreach attempts, and monitoring data in the EHR • Ensure documentation supports RPM and CCM program requirements • Maintain strict adherence to HIPAA and patient confidentiality standards Patient Outreach & Care Coordination • Conduct scheduled wellness check-ins and follow-ups with patients • Provide reminders and basic care coordination support • Document all patient communication clearly and compliantly Operational Excellence • Follow standardized workflows and escalation pathways • Meet established productivity and quality benchmarks • Participate in quality improvement initiatives • Support onboarding of new team members as needed Who Will Thrive in This Role • Someone who values structure, clarity, and process • A detail-oriented professional who takes documentation seriously • A strong communicator who is confident speaking with patients and clinical staff • Comfortable working independently in a remote environment • Accountable, reliable, and responsive • Motivated by precision and impact Qualifications • Active Certified Nursing Assistant (CNA) or Certified/Registered Medical Assistant (CMA/RMA) required • Minimum 1–2 years of healthcare experience (RPM, CCM, primary care, home health, or similar preferred) • Experience working in electronic health record (EHR) systems • Strong written and verbal communication skills • High attention to detail and ability to manage multiple priorities • Bilingual is a plus.
