As a Field HCBS Wait List Care Coordinator, you will be responsible for developing and implementing person-centered, goal-oriented care plans that address the unique needs of individuals receiving home and community-based services. Your expertise will ensure that our clients receive timely, effective, and cost-efficient care, while also promoting their independence and well-being.

- Local travel required (up to 75%), with mileage reimbursement at the current government rate
Primary Responsibilities:
- Assess, plan, and implement individualized care strategies that promote the most appropriate, least restrictive level of care
- Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
- Ensure care plans address social determinants of health, covered services, and value-added benefits
- Communicate effectively with stakeholders to ensure quality, coordinated care and services
- Advocate for individuals and families, ensuring their needs and choices are fully represented and supported
- Conduct home visits, in-person visits, and serve as a resource for community care coordinators

Required Qualifications:
- Bachelor's degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or a related human services field
- 1+ years of experience working with individuals with long-term care needs
- 1+ years of experience working in a community health setting in a healthcare role
- Intermediate level of computer proficiency, including Microsoft Outlook, Teams, and multiple web applications
- Ability to travel 75% of the time, with a valid driver's license, current automobile insurance, and reliable transportation
- Access to a designated quiet workspace in your home, with a secure internet connection
Preferred Qualifications:
- Licensed Social Worker or clinical degree
- Experience working with individuals receiving services on one of the home and community-based waivers in Kansas
- Background in managing populations with complex medical or behavioral needs
- Experience with electronic charting and arranging community resources
Soft Skills:
- Strong communication and customer service skills
- Ability to work independently, with good judgment and accountability
- Demonstrated ability to work well with others, with strong organizational and time management skills
- Ability to multi-task, prioritize tasks, and meet deadlines in a fast-paced environment
