Bilingual RN Care Manager (Remote Flexible - Spanish Speaking)

Pair Team is seeking a full-time Registered Nurse Care Manager to support patient-driven care plans and improve outcomes for individuals with Serious Mental Illness, Substance Use Disorder, homelessness, and/or high medical needs. The role involves working in a team-based model with other care professionals to provide health education, complex care management, medication reconciliation, care coordination, and clinical interventions. The position is primarily remote with 1-2 on-site community visits per month. Candidates must hold an active California RN license, be bilingual in English and Spanish, and have experience in care coordination or case management. The role requires strong technical skills, cultural fluency, empathy, and the ability to engage individuals with complex needs.
