Care Manager — LPN/CMA (Remote)
Looking for motivated LPNs (up to $24/hr) or CMAs (up to $17/hr) for a full-time, Monday–Friday, work-from-home Chronic Care Management role. You will provide one-on-one phone-based support to Medicare patients with two or more chronic conditions: assess risks, educate, encourage treatment adherence, and build patient-specific Care Plans in collaboration with the PCP. Requirements: LPN or CMA; high-speed (non-satellite) internet; proficiency in Outlook and Excel; EMR documentation familiarity; self-motivated and disciplined. Applicants must reside in FL, GA, AL, MS, TN, TX, or OK.
