Care Manager (BH Licensed, Remote in UT)
Licensed Social Worker Case Manager supporting Medicaid SMI & Substance Abuse Population in Utah, working remotely and participating in interdisciplinary care team meetings to ensure members have care plans based on their concerns/health needs.
Requirements
• 2 years health care experience, preferably in behavioral health
• Licensed behavioral health clinician (LCSW, APSW, CHES, LPC, LPCC, LMFT, PhD, or PsyD)
• Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance
• Experience with working with persons with severe and persistent mental health concerns and serious emotional disturbances, to include substance use disorder and foster care
