Pay Rate: $35.87/hour
Schedule: Monday–Friday, 8:00 AM–5:00 PM CST
Location: Fully Remote (Work from Home – Illinois only)
Position Overview
Help elevate patient care to a whole new level! Join a leading healthcare organization specializing in serving dual-eligible Medicare and Medicaid populations. As a Nurse Case Manager II, you’ll play an essential role in coordinating care for members facing complex medical and social challenges. Collaborate with members, providers, and community organizations to address the full continuum of health care and social determinant needs, driving positive outcomes through compassionate, data-driven, and cost-effective case management.
Key Responsibilities
• Conduct comprehensive assessments of members’ needs and eligibility using clinical tools and data review.
• Evaluate member benefit plans and available programs to determine the most appropriate care approach.
• Apply case management criteria, guidelines, and standardized care plans to ensure effective benefit administration.
• Facilitate communication and coordination between members, providers, and community resources to promote optimal health outcomes.
• Utilize quality management processes in accordance with regulatory and accreditation standards.
• Document case activity accurately in compliance with policies and procedures.
• Use multiple software systems efficiently to manage caseloads and maintain detailed member information.
• Collaborate virtually with care management teams and maintain a high degree of independence in decision-making.
Qualifications
• Licensure: Current, unrestricted RN, LCSW, or LCPC license in the state of Illinois (required).
Education: Verifiable High School Diploma or GED required; degree in Nursing or Social Work preferred.
Experience:
• Minimum 3–5 years of clinical experience (e.g., hospital, home health, or acute care).
• 2–3 years of care management, discharge planning, and/or home health coordination preferred.
• Prior experience with case management and Illinois waiver services is a plus.
• Skills:
• Strong analytical, problem-solving, and decision-making skills.
• Excellent written and verbal communication abilities.
• Ability to work independently and manage multiple priorities effectively.
• Proficiency with MS Word, Excel, Outlook, PowerPoint, and electronic case management systems.
• Confidence working remotely and collaborating virtually.
Other Requirements:
• Comfortable working fully remote (never onsite).

• Position Type: Temporary-to-Permanent (Extension or permanent hire possible based on performance and business needs).
• Work Hours: Monday–Friday, 8:00 AM–5:00 PM CST.
