**Experienced Licensed Social Worker – Remote Medical Social Work and Case Management**

This role requires researching available options and resources for members with financial, transportation, safety, and housing needs. The population spans the entire lifecycle, and all member contact is done telephonically or via electronic exchange. The successful candidate will work to empower members to make positive lifestyle and medical choices and will work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health. **Responsibilities** * Partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs * Research available options and resources for members with financial, transportation, safety, and housing needs * Empower members to make positive lifestyle and medical choices * Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health * Conduct telephonic or electronic exchanges with members to assess their needs and develop plans to meet those needs * Collaborate with other healthcare professionals to ensure comprehensive care and support for members * Maintain accurate and up-to-date records of member interactions and progress **Requirements** * Active and unrestricted LSW - Licensed Social Worker, LMSW - Licensed Masters Social Worker, LCSW - Licensed Clinical Social Worker, or LISW - Licensed Independent Social Worker in the state of residence * 2+ years of experience in medical social work or case management * Masters Degree in Social Work **Preferred Qualifications** * Experience working with diverse populations, including children, adults, and older adults * Knowledge of community resources and services for medical, financial, and behavioral health * Strong communication and interpersonal skills * Ability to work independently and as part of a team * Strong problem-solving and critical thinking skills **Work Environment and Culture** * This is a remote work-from-home role, and the successful candidate will be able to work from anywhere in the U.S. * The working hours will be Monday-Friday, 8am-4:30pm Pacific Standard Time, with no weekends or holidays required. * blithequark is a dynamic and fast-paced environment that values innovation, collaboration, and teamwork. * We offer a comprehensive benefits package, including medical, dental, and vision benefits, as well as a 401(k) retirement savings plan and an Employee Stock Purchase Plan. * We also offer numerous well-being programs, education assistance, free development courses, a CVS store discount, and discount programs with participating partners. **Compensation and Benefits** * The typical pay range for this role is $46,597.20 - $101,228.40, depending on experience, education, and other relevant factors. * This position is eligible for a blithequark bonus, commission, or short-term incentive program in addition to the base pay range listed above. * We offer a comprehensive benefits package, including medical, dental, and vision benefits, as well as a 401(k) retirement savings plan and an Employee Stock Purchase Plan. * We also offer numerous well-being programs, education assistance, free development courses, a CVS store discount, and discount programs with participating partners. **How to Apply** If you are a motivated and compassionate Licensed Social Worker looking for a new challenge in the healthcare industry, we encourage you to apply for this exciting opportunity.
